After your purchase, you will need to complete our clinical intake form for the medical provider to review your information.

One order is designed for ONE ADULT and intended for use only by the person for which it is prescribed for.

IMPORTANT: Immediately after kit purchase, please complete the required intake form sent to your email for one of our providers to evaluate. Once approved, expect to receive your kit within 2-3 weeks.

**Prescriptions are administered at the sole discretion of your provider. In the event of allergy or other medical condition that is a contraindication for a medication, it shall be omitted from the kit without substitution.
